Questionnaire for previous theists.

I'm no expert of the subject of marketing, it did however occur to me, that this website, in a coalition with some other atheist websites, could do a great deal of good research into what turns people atheist. We could then use this data to create more effective marketing and advertising.
The design of the questions and possible tick-box answers, could be designed by people in our community who have a background in research, psychology, marketing etc. The questionnaire when designed, could be placed on the banner part. or made a feature of as many websites as possible.
I was only ever a child theist, and not a very serious one at that. Theism was rarely discussed at home or school. Church only attended for harvest festival, christenings, weddings and funerals. I had stopped believing in god, by the time I was 12, around 4 years after I was sure there was no Santa. I don't really remember the crossing point, into atheism for me. So I would presume -- again I'm no expert on this -- that there would be little value in surveying me. The first questions could be such that we can determine the age at which atheist beliefs took hold, and how strong the religious conviction was before conversion to atheism. Further questions on what the possible trigger points were could give us some great data.
If you have any ideas for questions for this project. I'd like to hear them, the possible answers, and the reasons behind the questions. Bear in mind that to get the data out, we need multiple choice answers.
