I see two major concerns you raise in the 'free' versus 'free market' healthcare debate:
1. Quality of treatment.
2. Cost of treatment (through tax/insurance) to the patient.
These are both valid points - and important.
I am adding the 'fairness' concern. The trickiest of the lot...
I suppose my approach in life is to tend towards 'fairness' at the slight expense of, well, expense. Unlike FF, I'm pleased my tax pays for the treatment of people I may well never meet. (Yes, I am narked by how some of the money is spent, but no system is perfect). Society as a whole - of which I am part - benefits.
I am of the opinion that access to healthcare should be based on clinical need and not cost (to the patient). Others may have a different opinion.

Here in the UK, if a governemt introduced legislation so that a Doctor's visit - or visit to the Doctor - were chargeable, well, I wouldn't bet on them being re-elected. Free at point of use medical services in the UK were a hard won universal right that was opposed by the Doctors themselves. Why?
