I would go along with the conclusion that the “cause” is more psychological than hormonal, but I would hesitate to use the word “illness”, unless you’re willing to say that nearly EVERYBODY is psychologically ill to some degree or another.

Although I am trying to accept transgendered individuals, I am old-fashioned enough to ask them to cut us some slack.  Most people will automatically treat you as the gender you appear to be.  You can’t expect us to read your mind and guess what gender you’re feeling on any given day.  I recently read a novel in which one character was a transgender.  Although she appeared female, one any given day she might “feel” male and get upset if people didn’t address her by the masculine version of her name.  The other characters in the novel took this in stride as normal.  But for most of us, that would be too confusing.
